CONSENT TO THE PROCESSING OF PERSONAL DATA

I (hereinafter referred to as the "Personal Data Subject"), of my own free will and in my own interest, I give my voluntary, specific, substantive, informed, conscious, and unambiguous consent to the processing of my personal data to JSC GENERIUM (Russian Federation, 601125, Vladimir oblast, Petushinsky district, Volginsky settlement, Zavodskaya Street, building 273) (hereinafter referred to as the Operator) in accordance with the requirements of Federal law "On personal data" as of July, 27th 2006, No 152-FZ.
This consent is given by checking the box in the designated checkbox on the Operator's website.

I give consent to the Operator for the processing of my personal data on the following conditions:

Composition of personal data:

• Surname, first name, patronymic (if any);
• date of birth;
• phone number;
• gender;
• growth;
• weight;
• treatment (outpatient or inpatient);
• country of residence;
• residential address;
• email address;

Purpose of personal data processing:

• receiving and processing reports on adverse events that coincided with the use of a medicinal product and/or cases of lack of therapeutic efficacy, within the framework of fulfilling obligations under Pharmacovigilance, including communication with the subject to obtain clarifying information.

Actions (operations) with personal data:

• collection; recording; ordering, accumulation; storage; adjustment (updating or modifying); retrieval; usage; transfer to third parties (provision, access), including persons involved in the implementation of specified purposes (Operator's employees), in compliance with measures to protection of personal data from unauthorized access; blocking; removal; erasure.

Methods of personal data processing:

• with and without the use of automation tools.

I am informed and hereby confirm that the Operator does not verify the accuracy of the personal data provided by me. The Operator assumes that the User provides reliable and necessary (not excessive) personal data for the achievement of the specified processing purposes.
